The toc.ncx is a part of any epub (and also appears in an azw3 converted by Calibre from an epub-but I don't know if it is actually part of the azw3 spec. I always use the editor-I've had inconsistent results trying to do it at conversion, because you never know what is in the book for the conversion engine to work with. In the editor, I make sure each item I want in the table is its own separate html file, then I use the TOC editor to a) blow away anything that's already there, and b) create the table from files.
